3, Why M.P.H.?
Master of Public Health (M.P.H.) Program での―年について

私は、1994年秋から1995年春まで、The Johns Hopkins University,
School of Hygiene & PubIicHealthの M.P.H.プログラムにて勉強を
しました。
なんで麻酔科医がまたそんなPublic Healthなんかを、とお思いの先生方
もいらっしゃるかもしれませんね。少し背景を説明します。私は、米国へ
来る前に岐阜大学で働いていた時に、カンボジアヘ行き麻酔を教えたり、
多少のリサ-チを試みたりしました。その経験からInternational Health
も含めて、しかし、それだけでない、幅の広い勉強の必要性を感じ決心した
わけです。日本での臨床の忙しさに加えて、日本にはこちらのSchool of
Public Healthのように非常に広い分野の勉強をできるところはなかなか
ありません。とにかく、School of Public Healthがないのですから。
M.P.H.は、practicalなdegreeで、course workが主であり、thesisはなく、
どこの大学でも約一年間で取ることができます。 Johns Hopkinsの場合は、
7月から翌5月までの11か月(5学期)で、学費はおよそ1学期 $4,500ですから
年間$20,000以上にもなります。他のMaster(M.B.S.はたぶん除く)と異なり、
Heath関係のさまざまな分野で経験を積んでからはいって来るひとが多く、
また、私の年のクラス約100人のうち、半分ぐらいがPhysicianだったという
ことです。学生は、アジア、アフリカ、ヨーロッパ、etc.世界中から集まって
いました。
さて、homeworkと試験に明け暮れた―年で、私がどんな勉強をしたか、以下

当初、lnternational Healthを中心に勉強をと思っていたのですが、
Hopkinsのcourseは、私のやりたいことと違うような印象を受け、
Biostatistics, Epidemiology(特に、CIinicaIlnvestigation)でいろいろな
種類のリサーチの基礎と考え方を、また、Environmental HeaIthを学ぶことに
力を入れました。
HopkinsのBiostatisticsとEpidemiologyは、アメリカ中でかなり評価が高く、
Class roomでのteaching、laboratoryでのdiscussion、そしてassignments
など、非常によくorganizeされていてます。 BiostatisticsIこ関しては、
後程、別の稿で紹介させて頂こうと思っていますが、―年間のcourseで、
単純なprobabilityの話から、general linear、logistic regression,etc
までを、実際に十分に応用ができるようになるまでtrainingされます。
さて、M.P.H.を持ったひとが、実際どんな所で活躍しているかと言いますと、
UNなどの国際機関、NGO、各国のHealth Officersとして、また、様々な分野
でのリサーチ。 ちなみに、ANESTHESlOLOGY Editorial BoardのStatistician
はM.P.H.ということです。日本では、lnternational Health関係でM.P.H.を
取って活躍している方は多いと思いますが、他の分野ではどうでしょうか。
まだ、あまり、appreciateされていないのではないでしょうか。
しかしながら、麻酔関連分野においても、将来ICU,OR,ERなどでClinical trial
をやっていきたいという先生方、医療費の急増するこれらの分野において
cost effectivenessやmanagementにカを入れていきたいという先生方には、
おすすめのプログラムだと思います。
おそらく、持っていらっしゃったPublic Healthのイメージと少し違うとお思
いの方も多いのではないでしょうか。私自身がそういう印象を受けました。
何だか、医学、医療の基礎の基礎をたたきこまれたような、医学部における教育の
足りない所を全て考えさせられたような、そんな感じです。正直なところ私に
とっては、期待していた以上の勉強になりました。
これを私の将来のどう役立てるか、長くなりましたので、また、別の機会にさせて
いただきたいと思います。
